Start the Process in 5 Easy Steps

 

  1. Connect: An Axis for Autism team member will reach out within two business days of completing the form to determine if your loved one is eligible for an ADOS-2 evaluation. Get scheduled as soon as one week from contacting us.

  2. Prepare: Complete intake paperwork and questionnaires from the comfort of your home, or reach out if you need further assistance and we can set up a paperwork appointment to help the clinician better understand your child’s development.

  3. Evaluation: Visit one of our offices for one to two 45- to 90-minute appointments. Your child will take part in various activities while the clinician observes for autism symptoms. Our testing rooms are quiet, playful and designed to accommodate families.

  4. Follow-Up: Visit with a psychologist after two weeks via phone or telehealth to receive results, an action plan and an autism diagnosis, when appropriate.  

  5. Report: You will receive a written report and recommendations including formal documentation of the diagnosis, along with guidance and local resources for interventions and therapies.
