Find Answers in 5 Steps

 
  1. Evaluation Consult: An Axis for Autism team member will reach out within one business day of completing the form to determine if your loved one is eligible for an evaluation. Get scheduled as soon as one week from contacting us.

  2. Background: Complete intake paperwork and questionnaires from the comfort of your home, or reach out if you need further assistance and we can set up a paperwork appointment to help the clinician better understand your child’s development.

  3. Evaluation: Visit one of our offices for a 45- to 90-minute appointment. Your child will take part in various activities while the clinician observes. Our testing rooms are quiet, playful and designed to accommodate families.

  4. Follow-Up: Visit with a psychologist after two weeks via phone or computer to receive results, an action plan and a diagnosis, when appropriate.

  5. Report: You will receive a written report and recommendations including formal documentation of the diagnosis, along with guidance and local resources for interventions and therapies.
