ABA therapy at Axis requires an autism diagnosis and a provider referral. Here is how to begin, whether you evaluated with us or elsewhere.
What you need before starting.
To begin ABA services, families need two things: a documented autism diagnosis and a referral from a qualified provider. If your child was evaluated at Axis, our diagnostic and ABA teams coordinate so you do not repeat steps unnecessarily.
Autism diagnosis
A formal autism diagnosis from a qualified clinician is required. If you need an evaluation, our diagnostic team can help, often with shorter waits than elsewhere in Arizona.
Provider referral
A referral from a physician, psychologist, or other licensed provider is needed to begin ABA. Referring providers can submit referrals by fax or through our online form.

Verify insurance
We accept most Medicaid plans and offer private pay. Call with your insurance card and we will verify benefits and any authorization requirements.

Complete intake
Intake paperwork helps us understand your child's history, strengths, and family goals. Forms can be completed at home or in person.

Begin assessment and therapy
A comprehensive assessment informs individualized goals. Center-based sessions begin once authorization and scheduling are in place.
