Referral process
Send a referral by phone, fax, or webform. We respond within one business day and will guide you through next steps.
Three ways to refer.
Choose the method that works best for your practice. Our intake team will follow up within one business day.

Call
Reach our diagnostic intake line at (602) 888-8882 or our ABA intake line at (602) 888-8883, depending on the service you are referring for.

Fax
Send referral documentation and clinical records to (602) 883-7254. Include patient contact information and the reason for referral when available.

Webform
Submit the provider referral form online, in its diagnostic evaluation or ABA therapy version. Schools, agencies, and case managers have a separate community referral form.
Referring from a school, agency, or case management team instead? Use the community referral form. Families starting on their own are better served by Request an Evaluation or Request ABA Services.
Fax a referral
Fax referral forms and supporting clinical documentation to (602) 883-7254. Please include patient demographics, contact information, insurance details when available, and the reason for referral.
